Make sure the mpq is named Patch_D2.mpq, drop it into your Diablo II folder, remove the -direct -txt from your shortcut and there you have it.

Yeah I feel kind of dumb. So 17k runs later I realise that the -direct -txt is increasing the load time of the game by between 1-2 seconds. Ouch, ok who can I blame for not telling me this. :scratchchin: edit: thats 5-10 hours of extra loading to watch.
